A  mixture  of  these tribes is  the  product  of  tribal  mix   in  the  middle  mainly in Khartoum  and  gezera  states

Sudan  is  deeply  engaged  in  the  history  ,  the  traces  of  the  first  human beings

    Intervertibral disc prolapse usually affect lumbosacral spine but cervical and dorsal areas may be also affected .

    Intervertibral disc prolapse affecting the lumbosacral spine is a very common condition  which is usually associated with non professional muscular effort and presented with chronic low back ache and lower extremities pain , numbness , heaviness and  is sometimes associated with sphincteric changes in sever and advances cases , men are the usaual victims but females are also affected the lower levels of the lumbosacral spine are the usually affected levels ,

    The disc disease might affect one side or both sides according to the nature of the prolapsed disc whether to one side or central

    Disc porlapse of the cervical spine is characterized by symptoms affecting the upper extremities and  lower extremities like pain , numbness , heaviness beside neck and shoulder pain .

    Disc disease is a degenerative condition affecting the disc ,  leading to prolapse of the soft nucleus pulposus through it's fibrous container the anulous fibrosus mostly due to strains on the disc .

    The recent investigation to diagnose the intervertibral disc prolapse is by doing the magnetic resonance imaging of the affected area .

    Many doctors try to treat disc disease conservatively before trying to do any surgical interventions so we advise the patients to reduce their weight if they have an extra weight and or  to reduce their muscular effort and to have some rest , Analgesics may be prescribed together with lumber supporting corsets or cervical collars , and other may advise some certain exercises to strengthen back musceles .

    The above mentioned management results may be satisfactory in many patients  but in some of them this management fails and still the patients need an alternative method of treatment  .

    0000endoscopic discectomy4Laser or theromcoagulation of the disc is effective way in treating certain types of contained discs in some patients in this method the targeted disc is reached under X ray or an image intensifier guidance by a certain probe and then the disc is heated the result will be small shrinked  disc .

    Still many patients need surgical removal of the disc which is usually done through an open wound whether small one or large one , but traditional  disc surgery is in many cases associated with many problems , starting from long hospitalization to long rest  after surgery to other problems like the need for a long term big amounts of Analgesics , Antibiotics and sometimes the need for blood transfusion with it's hazards .

    Endoscopic discectomy is one of the most recent methods in treating disc prolapse , it make the use of the advances in endoscopic industry to make small skin incision and passage through muscles and other structure to reach the disc in it's place and put the endscopic eye close to the disc to see every thing in it's place.

    There are different devices and procedures but all of them have the same idea , these methods can be used with different types and levels of discs , but it is most popularly used in lumbosaral and cervical discs . nowadays the use of endoscopes has extended to involve decompression of spinal canal stenosis , spinal tumours and even spinal instrumentation the increment of the use of endoscopes in different spinal conditions is due to very advanced technology and the need for new methods of management to reduce the complications associated with traditional spinal surgery ,

    What made spinal endoscopy popualar is the reduced need for antibiotics , analgesics , the rapid recovery , the less need for blood transfusion , the cost of these great benefits is the need to provide new equipments and to train the treating personelles on these equipments
